Once a referral is received by our skilled nursing facility, our Admissions Coordinator will immediately establish communication with the referring facility to retrieve information for review related to our rehabilitation services.

Our admissions coordinator will evaluate your insurance information to confirm that we are in network with your policy for our rehabilitation services at the skilled nursing facility.

Our Director of Nursing will thoroughly evaluate clinical documentation to ensure that the admissions coordinator has all the necessary information for our rehabilitation services at the skilled nursing facility.

If, for any reason, our admissions coordinator is unable to accept you or your loved one into our skilled nursing facility, believe us, it breaks our hearts just as much as it may yours, especially when we know how important our rehabilitation services can be.
